Payments and Fees

All co-pays, deductibles, and co-insurances are due at the time of visit.

We reserve the right to discontinue our services if an account is past due more than 30 days.

For any urgent forms that need to be returned immediately, there will be a $10 fee.

Returned checks will incur a $25 charge.

Private visits are priced according to the complexity of the case. Please ask the front desk for more information.
